Smithville Elementary will hold Pre-K and Kindergarten registration on May 6 for the 2021-22 school year.
Pre-K registration will be conducted in drive through fashion from 8:30-10:30 am and 11:30 am-2pm. Completed registration paperwork will need to be returned to Michelle Burklow at the Board of Education.
Kindergarten registration will be held at the DeKalb County Complex and has specific enrollment times and are as follows:
Last name A-D: 8am-9am, E-H: 9am-10am, I-L: 10am-11am, M-Q – 11am-12pm, R-T: 12pm-1pm, U-Z: 1pm-2pm, Open 2pm-3pm.
Please bring COPIES of the following documents to registration:
Certified copy of birth certificate, current, up-to-date, physical examination and immunization record (these must be on a Tennessee Department of Health Certificate of Immunization, which can be obtained from your doctor’s office or the health department, and proof of residency for DeKalb County.
DeKalb West School’s Pre-K and Kindergarten registration will also be held May 6 from 8am-2pm. At the school.
DeKalb West’s Kindergarten Registration for the 2021-22 school year will be Thursday, May 6 from 8:00 a.m. until 2:00 p.m. at DeKalb West School.
Please come at the assigned time according to child’s last name:
Please bring COPIES of the following documents to Registration: Certified copy of birth certificate, Current, up-to-date, physical examination and immunization record-these must be on a Tennessee Department of Health Certificate of Immunization, which can be obtained from your doctor’s office or the Health Department, Proof of residency for DeKalb County (i.e. gas/electric bill), Your child (they will be tested on registration day)
Pre-K Registration forms for DeKalb West for the 2021-22 school year can be picked up on Thursday, May 6 from 8:00 a.m. until 2:00 p.m. at DeKalb West School. Completed registration paperwork will need to be returned to Michelle Burklow at the Board of Education office.
COPIES of the following documents must be submitted with your registration forms in order for it to be complete:
Certified copy of birth certificate, Current, up-to-date, physical examination and immunization record (These must be on a Tennessee Department of Health Certificate of Immunization, which can be obtained from your doctor’s office or the Health Department), Proof of residency for DeKalb County (i.e. gas/electric bill), Proof of income for all household members
Student entering Pre-K will also need proof of income for all household members, and Kindergarten students must be present at registration.
Children who register to attend Pre-K must turn four-years old on or before August 15, 2021, while children registering for Kindergarten must turn five-years old on or before August 15, 2021.
