In the second pageant of the evening, 12 year-old Keara McKinsey Milligan was crowned Miss Sweetheart queen. She is the daughter of Josh and Cyndi Milligan of Smithville. She also received the Most Photogenic award.

Milligan succeeds the 2018 Miss Sweetheart Addison Isabella Kyle. She is the 12 year- old daughter of Jamie Kyle and Cory and Kim Crabtree of Smithville.

The pageant featured eight young ladies ages 10-12.

Bailey Elizabeth Kidd, the 11 year-old daughter of Kelli Bess and Brandon Kidd of Smithville, was named first runner-up.

Madison Brooke Dawson received second runner-up. She is the twelve year-old daughter of Jarred and Jamie Dawson of Smithville.

Third runner-up went to 11 year old Averie Nicole McMinn. She is the 11 year-old daughter of Nathan and Amanda McMinn of Smithville. She was also named Miss Congeniality.

Danica Hope Collier was named fourth runner-up. She is the 11 year-old daughter of Matthew Collier of Alexandria and Britany George of Sparta.

Others participating in the pageant were Autumn Grace Dies, 11 year-old daughter of Keith and Amanda Dies of Alexandria; Hailee Ann Marie Merriman, 11 year-old daughter of Jake and Joyce Merriman of Smithville; and Kaylyn Isabelle Prichard, 10 year-old daughter of Andy and Chrissy Prichard of Liberty.
